The role requires a Bachelor’s in Social Work or Human Services with 2 years of supervised healthcare experience; MSW is preferred. This is a fulltime on-site position with equal opportunity employment.

Qualifications

• Bachelor’s degree in Social Work or Human Services with 2 years of supervised social work in a healthcare setting.
• MSW preferred; licensure or certification may be required by state.
• Ability to read technical procedures and policy manuals; effective at presenting information.

Responsabilités

• Assist residents in achieving the highest practicable level of self-care, independence and well-being.
• Develop care plans for social services issues with residents and families.
• Provide emotional support and assist with coping with health conditions and loss.
• Coordinate outpatient appointments and on-site ancillary services.
• Discharge planning with agencies and interdisciplinary team.
• Document resident social service status and maintain records.
• Provide in-service training to staff as requested.
