Job summary

Responsibilities

• Assist Social Service Director in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Review and help update department policies and procedures annually.
• Assist in identifying residents' medically related social and emotional needs and coordinate services.
• Support discharge planning and coordination with community agencies.
