Qualifications

• Prior health-care experience preferred.
• Ability to read, analyze, and interpret general business material.
• Ability to write reports, business correspondence, and procedure manuals.
• Mathematical skills for fractions, percentages, ratios and proportions.
• Reasoning ability to solve practical problems and interpret instructions.

Responsibilities

• Assist Social Service Director in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff, and other departments to plan social services.
• Develop, administer, and coordinate department policies and procedures; review and update annually.
• Coordinate with other departments to ensure social services meet resident needs.
• Provide information to residents/families about Medicare/Medicaid and financial assistance.
• Record progress notes and monitor response to treatment plans; maintain confidentiality.
• Assist with discharge planning and coordinating outpatient services.
• Develop interdepartmental relationships to maintain social service program quality.
