Job summary

Qualifications

  • Must possess, at a minimum, a high school diploma or its equivalent.
  • Prior health-care experience preferred.
  • Ability to read, analyze, and interpret general business material and regulations.
  • Ability to write reports, correspondence, and procedures.
  • Ability to present information to managers and staff.

Responsibilities

  • Assist Social Service Director/Consultant in planning, developing, organizing, implementing, evaluating and directing social service programs.
  • Meet with administration, medical and nursing staff to plan social services.
  • Review and help update department policies and procedures.
  • Provide information to residents/families about Medicare/Medicaid and financial assistance.
  • Coordinate social service activities with other departments and community agencies.
  • Assist in discharge planning and arranging outpatient services as ordered.
