Qualifications

• High school diploma or equivalent required.
• Associate or Bachelor’s degree preferred but not required.
• Prior health-care experience preferred.

Responsabilités

• Assist in planning, developing, organizing, implementing, evaluating and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Develop, administer and coordinate department policies and procedures.
• Maintain regular social service progress notes and reports.
• Coordinate social services with other departments and community agencies.
• Assist with discharge planning and scheduling of on-site services.
• Provide residents and families with information on Medicare/Medicaid and financial assistance.
• Maintain confidentiality of resident information.
