Responsibilities

• Assist Social Service Director in planning, developing, organizing, implementing, evaluating and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Assist in developing and implementing policies for identifying medically related social and emotional needs.
• Participate in discharge planning and development of social care plans and resident assessments.
• Coordinate social service activities with other departments as necessary.
• Involve resident/family in planning social service programs where possible.
• Assist in obtaining resources from community social, health and welfare agencies.
• Orient residents to the long-term care environment and facilitate adjustment.
