Job summary

Senior Lifestyle is seeking a Social Services Assistant to help assess and address residents’ psychosocial needs and coordinate care with an interdisciplinary team. You will support the admission process, advocate for resident rights, and liaise with community agencies to ensure high-quality service for residents and families.

The role requires a Bachelor’s degree or equivalent experience, fluency in English, and basic computer skills.

Qualifications

• Bachelor’s degree from a four-year college or university or equivalent combination of education and experience.
• Fluency in English to speak, read and write with residents, families, and staff.
• Experience in Social Services, Psychology, Social Work, Counseling, or related field.

Responsibilities

• Assist with assessments and identify residents' psychosocial needs and facilitate meeting them.
• Promote staff sensitivity by educating staff on residents' feelings and behaviors when needed.
• Assist with MDS completion and care plan documentation within required timelines.
• Support the social care component of the Comprehensive Care Plan with the interdisciplinary team.
• Liaise with social, health, and community agencies to support residents.
