Qualifications

• Bachelor's degree in social services or related field.
• 1+ years experience in social services, psychology, or counseling.
• Fluent English communication in speaking, reading and writing.
• Basic computer skills for documentation and data entry.

Responsibilities

• Assist with assessments and identify residents' psychosocial needs.
• Assist with social care components of the Comprehensive Care Plan.
• Attend care plan meetings to provide guidance on residency advocacy and resident rights.
• Assist with admission processes and marketing to outside contacts.
• Contact families regarding resident needs and coordinate with social, health, and community agencies.
