Job summary

AA Healthcare Management is seeking a Social Services professional to plan, organize, implement, evaluate, and direct the social services department in a long-term care facility. The role ensures psychosocial and emotional needs are met for each resident in compliance with regulations.

The ideal candidate will hold a bachelor’s degree in a human services field and have at least one year of supervised experience in mental health or human services, contributing to policy development and quality

Qualifications

• Bachelor’s degree in a human services field (including but not limited to sociology, special education, rehabilitation counseling or psychology).
• Possess a minimum of one year of supervised experience in mental health or human services.

Responsibilities

• Review facility policies and procedures to assure compliance with state and federal regulations.
• Participate in developing facility social work policies and quality assurance meetings.
• Document progress in meeting residents’ psychosocial needs and support resident rights and wellbeing.
• Provide social history, psychosocial assessments, and interdisciplinary care plans for residents.
