Job summary

Senior Lifestyle is seeking a Social Services Assistant to support the Director of Social Services in delivering comprehensive social care for residents and their families. You will assist in assessments, MDS documentation, and developing care plans with the interdisciplinary team.

Your role includes liaising with hospitals and community agencies, supporting admissions, and helping staff respond to residents’ rights and needs.

Qualifications

• Bachelor’s degree or 1–2 years related experience required.
• Fluent English: speak, read, write and communicate effectively.
• Background in Social Services, Psychology, Social Work or Counseling preferred.

Responsibilities

• Assist with assessments and identify residents' psychosocial needs.
• Support MDS completion and documentation for the social care component of the Comprehensive Care Plan.
• Promote staff sensitivity to residents' needs through education.
• Liaise with social, health, and community agencies.
• Assist with admission processes and care plan meetings.
• Assist with marketing the facility and contacting families.
