Job summary

Covenant Living Communities and Services is seeking a Social Services Assistant to support residents across multiple levels of care. You will address emotional and psychological needs, participate in resident assessments, and facilitate inter-facility transfers as a resident advocate.

Responsibilities include coordinating with families, discharge planners, and the interdisciplinary team while maintaining documentation and compliance with regulations.

Qualifications

• Must have high school diploma or equivalent.
• Associates degree preferred.
• Certifications in related areas desirable.
• Entry level experience.

Responsibilities

• Represent the facility with prospective residents, families and discharge planners for skilled nursing care.
• Orient residents to the skilled nursing facility and services.
• Collect and record admission data including social, medical and financial information.
• Maintain current health record documentation related to social services in the care plan.
• Participate in resident care planning meetings and interdisciplinary communication.
• Maintain contact with hospital discharge planners for continuity of care and physician service.
• Assist residents/families in selecting new physicians when needed.
• Communicate admission or discharge information to departments.
• Ensure care plan documentation complies with regulations.
