Job summary

RHF's North Capital at Plymouth in Washington, DC seeks a Social Service Coordinator to support residents by assessing needs, coordinating services, and developing wellness programs for older adults in a 69-unit community.

The full-time, 40-hour-per-week position involves building relationships with local providers, advocating for residents, and maintaining records to ensure timely service delivery and improved quality of life.

Qualifications

• Bachelor’s degree in Gerontology, Human Services, Social Work, Psychology, Sociology, or related field.
• Experience working with diverse groups of seniors and disabled individuals preferred.

Responsibilities

• Assess residents' needs to identify services that can enhance independence.
• Coordinate connections with local service providers for health and wellness resources.
• Develop health and wellness programs tailored to residents.
• Maintain directory of community resources and services.
