Job summary

RHF is hiring a Part-Time Social Service Coordinator to support older adults in a 50-unit residence in Redding, CA. The role emphasizes resident assessment, service coordination, and program development to enhance independence and well-being.

You will build relationships with community providers, maintain resource directories, and advocate for residents, with a focus on crisis intervention and timely referrals. This is a 35-hour weekly, non-exempt position.

Qualifications

• Bachelor's degree preferred in Gerontology, Human Services, Social Work, Psychology, Sociology, or related field.
• Experience with diverse seniors and disabled individuals is preferred.
• Strong ethics and client confidentiality required.

Responsibilities

• Assess residents' needs and identify services to improve quality of life.
• Coordinate access to social, health, and wellness resources.
• Develop and sustain health and wellness programs.
• Maintain a directory of community resources and services.
• Advocate for residents and liaise with agencies and staff.
• Monitor referrals and document interactions in records.
