Job summary

United Church Homes, Inc. is seeking a Service Coordinator for Cherry Arbors to coordinate programs and services improving residents' independence and well-being.

This role focuses on linking residents to community resources, monitoring service quality, and maintaining detailed records. The position involves collaboration with families, staff, and service providers, promoting wellness, and arranging mobile healthcare and screenings.

Qualifications

• Bachelor’s degree in social work preferred.
• Associate degree in nursing, social work, or gerontology preferred (GUIDE NaviGuide).
• Two or more years of experience in social service delivery to the elderly population.
• Knowledge of community resources within the geographic service area.
• Experience with Alzheimer’s disease, dementia, or gerontology preferred.
• Computer skills: basic office software and internet proficiency.
• Driver’s license required; must meet vehicle insurance and driving record standards.

Responsibilities

• Assist and educate clients and families to access community services.
• Coordinate services with agencies, providers, and residents; monitor service delivery for timeliness and quality.
• Maintain resident files, intake, assessments, and referrals; document communications.
• Promote wellness activities and arrange onsite or mobile healthcare services and screenings.
• Coordinate educational events for residents, families, and staff about resources and eligibility.
• Create and update person-centered service plans and maintain records in the EMR.
• Travel as needed for visits, training, and events.
