Qualifications

• Two or more years’ experience working with social service delivery to the elderly population.
• Working knowledge of resources available in the geographic location serving the community.
• Proven experience in service management or facilitation.

Responsibilities

• Coordinate programs and services to assist clients in maintaining independence.
• Act as a liaison between community agencies, service providers and clients.
• Monitor the delivery of services to ensure they are appropriate and timely.
