Job summary

HumanGood in Aston Township, PA seeks a Social Services Coordinator who will organize, implement and evaluate the Social Services program to meet residents' psycho-social needs, including assessments and linking residents to supportive services to help them live independently in the community.

This full-time role runs Monday to Friday, 9:00 AM to 4:00 PM, with a pay range of $23.00–$25.00 per hour, depending on experience.

Qualifications

• 1+ years of experience in case management required.
• 2+ years of social services experience, preferably in a senior care setting.
• Bachelor’s Degree in social work or related program such as gerontology, psychology, or recreation therapy.
• Social Services certification preferred.

Responsibilities

• Assessing resident needs based on admission social services history, resident’s health, psychological and social needs
• Promoting a positive living experience for our residents
• Ensuring confidentiality by adhering to HIPPA & all other privacy standards
• Promoting a positive work environment by collaborating with community team members
