Job summary

National Church Residences in Alliance, OH is seeking a Service Coordinator I to coordinate programs and services that help participants maintain a good quality of life and age in place. The role reports to the Property Manager/Program Manager and requires a Bachelor’s degree with 3-5 years of relevant experience.

Responsibilities include liaising with community providers, organizing educational events, assessing participants, and documenting service delivery.

Qualifications

• Bachelor’s degree in social service or related field with 3–5 years’ experience serving elderly and disabled.
• Strong communication, comprehension and interpersonal skills required.
• Must have basic computer literacy and pass a literacy test (Windows, MS Office, web).
• Willingness to travel occasionally as directed by contract and department requests.

Responsibilities

• Coordinate programs and services to help participants maintain quality of life and age in place.
• Liaise with community agencies and identify low-cost services and discounts.
• Document services and maintain a Resource Directory of providers.
• Assist with care planning, assessments and referrals and ensure follow-up as required.
• Engage in outreach and education activities and monitor participant progress.
