Job summary

Wesley Woods Senior Living is seeking a Service Coordinator to connect residents with community services, conduct intake, and coordinate case management to support aging in place. You will build a network of providers, educate families, train residents, and maintain regulatory knowledge.

Qualifications

• College degree in Social Work, Gerontology, Psychology or Counseling.
• Two or more years of experience in social services delivery, preferably with seniors.
• Knowledge of aging processes and elder services.
• Basic knowledge of Medicare and supplemental insurance programs.
• Proficient in Microsoft Office.
• Strong written and verbal English communication.
• Ability to multi-task and stay organized.
• Supervisory or management experience preferred.

Responsibilities

• Provide general case management, intake, and referral services for residents.
• Establish linkages with community service providers and negotiate pricing.
• Build a provider directory for staff and residents.
• Develop collaborative networks with families, volunteers, and community resources.
• Refer residents to service providers for case management, personal assistance, and transportation.
• Develop case plans in coordination with assessments.
• Educate management on aging-in-place issues and service coordination.
• Train residents on their obligations and available services.
• Assist residents in building informal support networks.
• Stay current on federal and state regulations and attend ongoing education.
