Job summary

Wesley Living in Memphis is seeking a Service Coordinator to support seniors in our Graceland Gardens community. This full-time role includes conducting assessments, coordinating services, organizing wellness and educational programs, and providing crisis intervention while maintaining confidentiality.

You will advocate for residents, help them access programs like SNAP or Medicaid, and collaborate with local providers to improve aging-in-place outcomes.

Qualifications

• Bachelor’s degree or higher in human services field.
• Driver’s license required.

Responsibilities

• Case Management & Advocacy: conduct initial and ongoing functional assessments; help residents apply for programs like SNAP, Medicaid, and utility assistance.
• Service Coordination: establish and maintain relationships with local healthcare providers, home health agencies, and transportation services.
• Programming & Education: organize wellness clinics, financial literacy workshops, and social events.
• Crisis Intervention: provide immediate support and referrals during emergencies or transitions.
• Compliance & Documentation: maintain accurate, confidential resident files per HUD guidelines and privacy laws.
• Mediation: assist in resolving conflicts between residents or with management.
