Job summary

Community Housing Innovations, Inc. (CHI) seeks a Case Manager to develop and execute service plans in a supportive, empowering setting.

You will coordinate direct services, referrals, and role-model best practices to foster stabilization and independence for residents. Responsibilities include weekly progress reviews, coordination with social workers, and discharge planning to sustain benefits and community links.

Qualifications

• Education: High School Diploma or GED required; Bachelor’s degree preferred.
• Experience: 2+ years in case management, counseling, or advocacy in social services.
• English proficiency required; multilingual abilities are a plus.

Responsibilities

• Develop and maintain service and care plans for residents.
• Coordinate direct services and referrals to external providers.
• Meet residents weekly to review progress and update plans.
• Ensure residents enroll in public benefits and DHS programs as applicable.
• Document interactions in CARES and AWARDS systems.
