Job summary

Community Housing Innovations, Inc. (CHI) is seeking a full-time Case Manager for the Smithtown location to support participants toward independent housing and economic stability, providing direct services, referrals, and advocacy.

Qualifications include a Bachelor's in human services or related field, a Master’s in Social Work preferred, and two years in homelessness or housing. English proficiency is required; Spanish is a plus.

Qualifications

• Bachelor's degree in human services or related field.
• Master's degree in Social Work preferred.
• Two years’ experience in homelessness or housing preferred.

Responsibilities

• Adhere to program regulations and ensure quality services.
• Assist clients in developing an Independent Living Plan (ILP) and track progress.
• Provide information, referrals, counseling, crisis intervention, direct service, and advocacy.
• Develop and maintain a resource network and collaborate with DSS and other providers.
• Assist clients with budgets, resumes, and ADL skills as appropriate.
• Face-to-face meetings with clients.
• Inspect living spaces to assess ADL needs and housing compliance.
• Limit and document improper use of agency resources by clients.
• Evaluate and inform supervisor of housing unit needs.
• Complete discharge plans and timely reporting; maintain charts and files.
