Job summary

Altoona Health & Rehab in Altoona, AL is seeking a Resident Services Assistant to support admissions, appointments, and discharge planning while serving as a welcoming point of contact for residents and families.

You will assist with documentation, coordinate care across the team, and help ensure smooth transitions of care. A CNA is preferred but not required, and employment includes background checks and drug screening.

Qualifications

• Experience assisting with admissions and discharge planning.
• Ability to document accurately per facility policies.
• Background check and drug screening will be conducted.

Responsibilities

• Assist the Resident Services Coordinator with processing admission paperwork and meeting with residents and families to gather required information.
• Help complete and document social services assessments, including follow-up throughout a resident's stay.
• Support discharge planning by helping coordinate paperwork, services, resources, and communication with residents and families.
• Schedule and manage resident appointments, including communicating appointment details and coordinating transportation as needed.
• Work with nurses, therapists, and other team members to address resident needs and support smooth transitions of care.
• Participate in care conferences and help follow up on resident and family questions or concerns.
• Maintain accurate, timely documentation in accordance with facility policies and applicable requirements.
• Advocate for residents' rights, preferences, and dignity in every interaction.
• Keep the Resident Services Coordinator informed of upcoming appointments, discharge needs, incomplete paperwork, and concerns requiring follow-up.
