Qualifications

• Bachelor of Social Work, Gerontology, Psychology, Counseling, or equivalent combination of education, training and experience.
• 2-3 years of progressively responsible experience in social service delivery.
• Experience with PC based word processing and spreadsheet software.
• Demonstrated ability to advocate, organize, problem-solve and provide results for the population(s) served.
• Able to work a varied schedule including evenings and weekends; valid Driver’s License in state of residence; fluent in English; Spanish a plus.

Responsibilities

• Resident Case Management: ensure intake, annual assessments, referrals, evaluate health and social needs, and tailor service plans.
• Maintain records of resident interactions, goals and achievements.
• Follow up on lease violations to reduce evictions.
• Resident Programming and Engagement: work with managers and teams to provide services according to the resident services plan and standards.
• Develop and implement resident engagement activities and regular onsite programs to meet residents' needs.
• Build partnerships with community-based organizations for referrals and onsite services; provide direct services when external options are unavailable.
• Accurately collect and record data to measure resident and program outcomes.
