Qualifications

• Bachelor-level degree in social work, gerontology, psychology, counseling or equivalent.
• 2-3 years of progressively responsible experience in social service delivery.
• Demonstrated ability to advocate, organize, problem-solve and deliver results.
• Ability to work evenings and weekends as required.
• Valid driver's license in state of residence.

Responsibilities

• Coordinate resident intake, annual assessments, referrals and service plans to support resident stability.
• Maintain documentation of resident interactions, goals and outcomes.
• Collaborate with property management to align services with the resident services plan.
