Job summary

Boston Senior Home Care is seeking a Per Diem Supportive Housing Tenant Resource Counselor to work on-site in Supportive Housing sites. This part-time role supports residents to age in place, coordinates services, and links tenants with community resources.

The PDTRC also assists with appointments, transportation, translations, and tenancy-related issues to help prevent eviction. The position is limited to 780 hours per year (not to exceed 15 hours per week).

Qualifications

• BS or BA degree required in sociology, social work, gerontology or related field.
• Six months or more years of experience in elder care case management required.

Responsibilities

• Serve as a resource person for the assigned Supportive Housing building by providing knowledge on consumer and government resources to all residents.
• Develop relationships with on-site partners including building manager and maintenance.
• Conduct trainings for residents on Evidence Based classes.
• Disseminate information to residents via monthly calendars and newsletters in multiple languages.
• Maintain a Supportive Housing office ensuring confidentiality of information.
• Collect and analyze data for program improvement and reporting.
• Plan and direct on- and off-site social activities and workshops.
• Document resident interactions and maintain files per BSCH standards.
• Perform other duties as assigned by Director of Community Housing.
