Job summary

MOUNTAINVALLEY is seeking a Staff Physical Therapist to evaluate and treat residents, communicate with families, physicians and health team members, and maintain documentation of services in medical records.

You will supervise PT assistants, collaborate with the care team, and educate residents and families on treatment goals and home programs, ensuring safe, compliant, and high-quality care.

Qualifications

• Bachelor's degree in Physical Therapy; prefer graduates of Master's or Doctorate Program in Physical Therapy.
• Licensed Physical Therapist in the state; must maintain license in good standing.
• Proficient with Net Health and Point Click Care software.

Responsibilities

• Evaluate residents to determine physical therapy needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and ensure compliance with state practice acts.
• Communicate progress, goals, and discharge plans with physicians, nursing staff and families.
