Job summary

Responsibilities

• Evaluate patients within 24 hours of referral.
• Develop and implement treatment plans with physician approval.
• Provide therapy and monitor patient progress.
• Maintain accurate and timely documentation.
• Communicate with physicians, nursing staff, and team members.
• Supervise PTAs, aides, and students.
• Participate in care planning, meetings, and discharge planning.
• Educate patients and caregivers on ongoing care needs.
• Recommend appropriate durable medical equipment.
