Job summary

Riderwood Village by Erickson Senior Living is hiring a resident-centered Physical Therapist with Home Care experience to join our Certified Home Health team. You will implement residents' personal care plans on a campus setting, emphasizing comfort and well-being for older adults across care continuum.

Must hold a Maryland Physical Therapy license and meet Medicare/state requirements. Interdisciplinary collaboration and growth opportunities are emphasized within a supportive campus community.

Qualifications

• Must meet current Medicare and Maryland requirements.
• Must possess a valid Maryland State Physical Therapy license.
• Experience completing documentation for reimbursement/regulatory requirements.
• Home Care Physical Therapy experience required.
• Therapeutic experience with older adults across multiple diagnostic groups preferred.

Responsibilities

• Provide holistic PT services across physical, spiritual, emotional, and clinical needs.
• Implement residents’ personal care plans with focus on strengths and routines.
• Collaborate with interdisciplinary team to address residents' needs throughout care.
