Location

Position

Physical Therapist (PRN) in Home Care for our Certified Home Health team. Join our rehab program and support residents in their daily living and continuity of care.

Compensation

$55.00 per hour plus a $5,000 sign‑on bonus. Note: the sign‑on bonus is not available to candidates sourced by third‑party agencies or internal candidates.

Responsibilities

• Provide personalized rehabilitation care to older adults across multiple diagnostic groups.
• Develop and implement resident care plans focusing on residents’ strengths, preferences and routines.
• Support the interdisciplinary clinical team to meet residents’ physical, spiritual, emotional and clinical needs.
• Document care to meet reimbursement and regulatory requirements (e.g., OASIS).

Qualifications

• Current Maryland State Physical Therapy license.
• Must meet Medicare and state requirements.
• Previous Home Care Physical Therapy experience required.
• Therapeutic experience working with older adults across multiple diagnostic groups is preferred.
