SEM Haven, Inc. is seeking a Care Coordinator to work cooperatively with residents, families and staff to create a positive, respectful home environment. You will provide personal care and support resident independence within our resident-centered, team-based approach.

You will help with daily living activities, attend household meetings, and assist with tasks such as meal support, tidying, and transportation of residents, always upholding our commitment to dignity and quality of life.

Qualifications

• One year experience working with the elderly preferred.
• Must be able to speak, read and write English.
• Freedom from illegal use of drugs; drug-free workplace required.

Responsabilités

• Deliver personal care to residents according to policies and procedures.
• Promote resident engagement and independence.
• Participate in resident-centered care and team-based management.
• Assist with food preparation and serving; cleanup; transporting residents.
• Attend household meetings and learning circles; fulfill simple resident requests.
