Qualifications

• High school diploma or equivalent required or preferred.
• Minimum one year in long-term care preferred.
• Ability to assess resident needs and coordinate discharge planning.

Responsibilities

• Agree not to disclose resident’s protected health information and report violations.
• Meet with resident and family to review policies on advance directives and rights.
• Conduct and document a social services evaluation identifying resident needs.
• Provide or arrange social work services as indicated.
• Assist with discharge planning and transfer coordination.
• Communicate with facility staff about resident issues and care plans.
