Qualifications

• Experience in social services or caregiving preferred.

Responsibilities

• Resident advocacy - act as a liaison between residents, staff and families.
• Emotional and psychological support - provide support to residents dealing with challenges, anxiety, mental health, etc.
• Discharge planning - assist residents and families with transitioning home and arranging support.
• Work with staff to ensure correct care for patients.
• All duties assigned by the director.
