Job summary

Forest Hill Health & Rehabilitation is seeking a compassionate Social Services Assistant to support residents in a long-term care setting. You will assess social needs, help develop care plans, and coordinate community resources while providing emotional support to residents and families.

The role emphasizes teamwork, patient-centered approaches, and collaboration with an interdisciplinary team to ensure holistic care and smooth discharge planning as needed.

Qualifications

• Bachelor’s degree in Social Work, Human Services, or related field preferred (Sociology, Psychology, Counseling, etc.)
• Experience in long-term care or healthcare setting preferred
• Strong communication and interpersonal skills
• Compassionate, patient-centered approach
• Ability to work effectively as part of a team
• Knowledge of community resources and social service programs is a plus

Responsibilities

• Assist in assessing residents’ social, emotional, and psychosocial needs
• Support the development and implementation of individualized care plans
• Provide emotional support and guidance to residents and families
• Help coordinate community resources and services for residents
• Maintain accurate documentation of social service interactions and care plans
• Collaborate with the interdisciplinary team to ensure holistic resident care
• Assist with care conferences and discharge planning as needed
• Observe and report changes in residents’ behavior or emotional well-being
• Promote resident dignity, independence, and quality of life
