Job summary

Tampa facility seeks a Social Services Coordinator to assist in planning, developing, and directing social service programs for residents. Qualifications

• Requires a high school diploma or equivalent
• Associate or Bachelor's Degree preferred but not required
• Prior health-care experience preferred

Responsibilities

• Assist in planning and directing social service programs.
• Meet with administration and medical staff to plan services.
• Develop and coordinate department policies.
• Identify resident social and emotional needs and plancare accordingly.
• Coordinate with other departments for service delivery.
• Discharge planning and community resource liaison.
• Maintain regular progress notes and documentation.
• Provide information on Medicare/Medicaid and financial aid.
• Assist in department budget development and monitoring.
• Ensure confidentiality of resident information.
