Job summary

Covenant Living Communities and Services is seeking a Social Services Assistant to support residents and families across multiple levels of care. You will participate in assessments, documentations, and inter-facility transfers while advocating for residents with the interdisciplinary team.

Responsibilities include coordinating admissions, assisting with orientation, and maintaining documentation for the care plan. A compassionate communicator with strong organizational skills will thrive here.

Qualifications

• High School Diploma or equivalent required.
• Associates degree preferred.
• Certifications in related areas are highly desirable.
• Entry level experience preferred.

Responsibilities

• Direct social services functions for residents and families across levels of care.
• Participate in resident assessment process and inter-facility transfers.
• Advocate for residents and document admission data and care plans.
• Assist in care planning meetings and ensure regulatory documentation is complete.
• Coordinate with hospital discharge planners for continuity of care.
