Job summary

PACS is seeking a Social Service Assistant to support the planning, development, and coordination of social services within the facility. You will collaborate with administration, medical and nursing staff to ensure comprehensive resident care and program effectiveness.

The role involves policy review, discharge planning, and assisting residents and families with resources while maintaining confidentiality and quality standards.

Qualifications

• Must have a high school diploma or equivalent; an Associate or Bachelor’s Degree is preferred but not required.
• Prior health care experience is preferred.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Review and update department policies and procedures; participate in changes.
• Identify medically related social and emotional needs and coordinate with relevant departments.
• Assist residents in achieving the highest practicable level of self-care and well-being.
• Coordinate discharge planning and outpatient referrals as needed.
• Prepare forms and reports; maintain progress notes and confidentiality.
