Job summary

PACS seeks a qualified social worker to support residents in post-acute settings. You will help residents and families explore options, set goals, and pursue the highest practicable level of independence and well-being.

You will evaluate psychosocial needs, develop care plans, provide emotional support, and coordinate with healthcare teams to secure resources and services for discharge planning and outpatient follow-up.

Qualifications

• Bachelor’s Degree in Social Work or in Human Services; MSW preferred.
• Strong communication and counseling skills; ability to interact with residents and families.

Responsibilities

• Establishes course of action by exploring options and setting goals with residents and their families.
• Assist residents in achieving the highest practicable level of self-care, independence and well-being.
• Provide medically related social services to attain or maintain the highest practicable physical, mental and psychosocial wellbeing.
• Evaluate social and family information and psychosocial needs to develop care plans.
• Provide emotional support and address emotional problems including coping with disabilities, health fears, and loss.
