Job summary

Ridgecrest Post Acute in Phoenix, AZ is seeking a Staff Physical Therapist Assistant to assist the Staff Physical Therapist with resident-related activities and direct resident care.

The position requires an Associate Degree, state PTA license, and proficiency with Net Health and PointClickCare for documentation. You will implement treatment plans, educate residents and families, and support quality improvement initiatives in an on-site setting.

Qualifications

• Associate degree from an accredited PTA program.
• Licensed PTA in the state and maintain licensure in good standing.
• Proficient in documentation systems (Net Health, PointClickCare).
• Ability to read and interpret policies and procedures.

Responsibilities

• Treat residents as directed by the Physical Therapist.
• Complete timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements.
• Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols.
• Communicates routinely with the supervising physical therapist to ensure timely updates to physicians, nursing staff, the interdisciplinary team, residents, and families regarding progress, goals, and discharge planning.
• Educate residents, families, and caregivers on treatment goals, home exercise programs, maintenance programs, safety techniques, and fall prevention strategies.
• Assist nursing department with training of Restorative Aides.
• Contribute to quality assurance, performance improvement initiatives, and facility rehabilitation program development as assigned.
