Job summary

PACS is seeking a Staff Physical Therapist Assistant to support the licensed Physical Therapist in resident care at our facility. You will assist with treatment planning, documentation, and equipment needs to enhance mobility and safety for residents.

The role requires an Associate Degree from an accredited PTA program, licensure, and proficiency with Net Health and PCC. Strong communication with the care team and families is essential in this position.

Qualifications

• PTA license required in the state of practice.
• Proficiency with Net Health and PCC data entry.

Responsibilities

• Treat residents as directed by the Physical Therapist.
• Complete timely, accurate documentation per regulations.
• Maintain a safe treatment environment via infection control and equipment maintenance.
• Participate in rehabilitation meetings and communicate progress to the team.
• Educate residents and families on treatment goals and home exercise programs.
• Assist in ordering and facilitating necessary durable medical equipment.
• Maintain confidentiality of residents and company information.
• Provide updates to physicians, nursing staff and families about progress and discharge planning.
• Lead staff in-services as needed.
• Assist training Restorative Aides.
