Job summary

You will coordinate with physicians, families, and the health team while mentoring PT assistants and staff.

Responsibilities include documentation, in‑services, training for facility staff, participation in care conferences, and ensuring infection control and safety.

Qualifications

• Bachelor's degree in Physical Therapy; advanced degrees preferred.
• Licensed Physical Therapist in the state; CPR certification may be required.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and aides in resident care.
• Educate residents, families, and caregivers on goals and home programs.
• Maintain compliant documentation in accordance with facility and payer requirements.
• Participate in care conferences and rehabilitation meetings.
• Maintain infection control and safety protocols.
