Salinas Valley Post Acute seeks an Assistant Director of Rehabilitation (Physical Therapist) to lead the therapy team, evaluate residents, and ensure high-quality post-acute care. The role combines clinical excellence with department leadership in a full-time capacity.

Responsibilities include supervising PTAs and staff, coordinating treatment plans, and ensuring compliant documentation while collaborating with physicians, families, and the interdisciplinary team to maximize resident outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and unrestricted Physical Therapist (PT) license in the State of California.
• Experience in a skilled nursing facility, post-acute care, or long-term care setting preferred.
• Leadership or supervisory experience preferred.
• Strong clinical assessment, documentation, and care planning skills.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans focused on mobility and independence.
• Assist nursing with Restorative Nursing Aides oversight.
• Supervise PTAs, rehab aides, and students following state regulations.
• Communicate progress with physicians, nursing staff, residents, and families.
• Complete timely, accurate clinical documentation compliant with standards.
