Hayward Gardens Post Acute, located in Hayward, CA, seeks a dedicated Physical Therapist (PT) to join our rehabilitation team. You will evaluate and treat residents, develop individualized plans, and collaborate with residents, families, physicians, and the interdisciplinary team to improve mobility, independence, and quality of life.

This role emphasizes resident-centered care, clinical excellence, teamwork, and professional growth through ongoing education and interdisciplinary collaboration.

Qualifications

• Current Physical Therapist license in the applicable state.
• Strong clinical reasoning and communication skills.
• Ability to supervise PTAs in accordance with state regulations.
• Commitment to compassionate, resident-centered care.

Responsabilités

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Provide skilled physical therapy interventions and monitor progress toward goals.
• Supervise Physical Therapist Assistants (PTAs).
• Assist nursing with Restorative Aide training.
• Communicate progress with physicians, families, and the interdisciplinary team.
• Participate in care conferences and rehabilitation meetings.
• Maintain compliant documentation and educate residents/families.
• Recommend durable medical equipment to promote safe mobility.
• Maintain a safe treatment environment and report equipment concerns.
