Qualifications

• Bachelor's degree in Physical Therapy required; Masters/Doctorate preferred.
• Proficient with data entry in Net Health and Point Click Care.
• Must be licensed as a Physical Therapist in the state.

Responsibilities

• Evaluate residents to determine PT needs, functional limitations, and rehab potential.
• Develop and update individualized treatment plans to improve mobility, strength, balance, endurance, and independence.
• Supervise PT assistants and support staff in resident care and activities.
• Communicate progress and discharge plans with physicians, nursing staff, residents and families.
• Complete timely, accurate documentation in accordance with policies and payer requirements.
• Maintain a safe treatment environment by following infection control and safety protocols.
