Qualifications

• Licensed as a Physical Therapist in the state.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Supervise PT assistants and staff per state practice acts.
• Communicate progress and discharge planning with physicians, nurses, residents and families.
• Complete timely and compliant documentation per facility and payer requirements.
• Maintain safe treatment environment with infection control and equipment protocols.
