Job summary

The role requires a Physical Therapy license in the state, with CE requirements and CPR certification if required. Proficiency with Net Health and Point Click Care is a plus.

Qualifications

• Licensed as a Physical Therapist in the state.
• Continuing education requirements must be met.
• Maintain license in good standing with the state board.
• CPR certification, if required by licensure.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and provide in-services as needed.
• Communicate with physicians, staff, residents, and families regarding progress.
• Complete timely and compliant documentation per regulations.
