Job summary

Cedar Crest Village by Erickson Senior Living is seeking a resident-centered Physical Therapist for a per diem role on our beautiful campus in Pompton Plains, NJ. You will provide physical therapy across inpatient and outpatient settings, work with residents to implement personal care plans, and collaborate with an interdisciplinary team to support holistic resident well-being.

Responsibilities include documenting to meet reimbursement and regulatory requirements while focusing on older adults’

Qualifications

• Must possess a valid and current state Physical Therapy license. Degree and training must meet current Medicare and state requirements.
• The ability to complete documentation to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.

Responsibilities

• Provide resident-centered physical therapy across inpatient and outpatient services.
• Implement residents’ personal care plans focusing on strengths, preferences, and routines.
• Collaborate with interdisciplinary team to meet residents’ needs and goals.
• Support documentation for reimbursement and regulatory compliance.
