You will implement personal care plans, focusing on residents' strengths and preferences, while maintaining regulatory compliance and documentation for reimbursement. Licensure and experience with older adults preferred.

Qualifications

• Must possess a valid and current state Physical Therapy license.
• Documentation ability to meet reimbursement and regulatory requirements.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.

Responsibilities

• Implement residents' personal care plans with a holistic approach focused on strengths, preferences and routines.
• Provide care across inpatient and outpatient settings as part of an interdisciplinary team.
