Qualifications

• Minimum of 1 year of experience in social services skilled nursing setting.
• Minimum of 1 year of supervisory experience in skilled nursing.
• Excellent English language usage in writing, speaking and presenting.
• Proficiency with Microsoft Word, Excel, and PowerPoint.
• Knowledge of Point Click Care a plus.

Responsibilities

• Complete all required social services charting to meet licensing requirements and reflect resident needs.
• Discharging, appointment setting and arranging transportation for residents.
• Managing, scheduling, mentoring and hiring staff.
• Adhering to the facilities operational budget.
• Participate as a member of the Interdisciplinary Team (IDT).
• Participating in facility surveys.
• Maintaining the social service directory and QA plans as warranted.
• Staying updated with laws and regulations pertaining to resident care.
• Following infection control and safety procedures.
