Burbank Healthcare and Rehabilitation Center is seeking a Social Services Director to join our interdisciplinary management team. The Director will supervise an assistant and foster a climate that maximizes residents’ independence, dignity, and quality of life in a long-term care setting.

The role includes policy development, regulatory compliance, advocacy for resident rights, and collaboration with families and staff to support psychosocial needs.

Qualificações

• Knowledge of Long Term Care facilities.
• At least 2 years of Social Services experience.

Responsabilidades

• Review facility policies and procedures as part of the interdisciplinary team to ensure compliance with regulations.
• Develop and review policies concerning resident care and quality of life.
• Develop and maintain facility social work policies and procedures.
• Create and maintain a directory of community resources for residents and families.
• Participate in quality assurance interdisciplinary meetings.
• Understand and document government requirements for social service records.
• Document progress in addressing residents' psychosocial needs.
• Advocate for resident rights and wellbeing with the team and administration.
• Identify community changes and programs affecting residents and provide updates.
• Coordinate with residents and families to support self-advocacy and quality of care.
