Laguna Hills Health and Rehabilitation is seeking a Director of Social Services to plan, develop, implement and evaluate social services programs ensuring residents' emotional and social needs are met.

The Director will lead the social services team, coordinate with IDT, ensure accurate documentation, and maintain regulatory compliance while delivering compassionate care and dignity to residents.

Qualifications

• Bachelor’s Degree in Social Work or a related human services field plus one year of supervised social work experience in a health care setting or equivalent.
• Bachelor’s Degree in Social Work or Social Service Designee Certification with at least 1 year experience for facilities under 120 beds.

Responsabilités

• Complete required social services charting (MDS, psychosocial evaluations, histories, PCP entries) to meet licensing requirements.
• Discharge planning, appointment setting, and arranging transportation for residents.
• Manage, schedule, mentor, and hire social services staff; participate in the IDT; support facility surveys.
• Maintain social service directory and assist QA Committee with plans of correction when needed.
• Stay updated on laws and regulations related to resident care and HIPAA confidentiality.
